Senior Case Management in West Central Arkansas
Our Case Managers have special training to provide information to the seniors of West Central Arkansas. They also provide all the information seniors need about the services and benefits available in their area. Our mission is to help seniors stay independent and in their own homes for as long as possible. To that end, WCA Case Managers act as a single point of contact for all your care needs. Senior case management is a perfect way to ensure you remain independent for as long as possible.
As part of senior case management, we also help ensure that all your or your loved one’s physical, mental, emotional, and medical needs are met. Case Managers coordinate with seniors, their loved ones, and their medical team to streamline and centralize the care process. We’ll help evaluate your needs. Then, we help you apply for the services and benefits you qualify for. Finally, we also monitor the progress of your services every step of the way.
What Does Senior Case Management Look Like?
Care Assessment
Our Case Managers are a single point of contact for all your care needs. This ensures nothing falls through the cracks and that all your needs are being met. Your Case Manager is responsible for assessing the care you are currently receiving. They then match you to programs or services that fill any gaps in care.
We locate, coordinate, and monitor all necessary and appropriate medical or social services for our clients. We also work with seniors at risk of abuse, neglect, or exploitation. We’ll ensure you get the care and resources you need to remain safe and healthy as part of your senior case management.
After you request senior case management services, we will send a Case Manager to your home to assess your care needs. Based on the findings of the assessment, your Case Manager will recommend the appropriate services and programs.
Application Assistance
Your Case Manager will also help you apply for any and all services or benefits for which you qualify. We are happy to help you fill out any paperwork required to get you the care you need. We’ll walk you through the application process step by step, and answer any questions you or your loved ones may have along the way.
Additionally, as part of your senior case management, a Case Manager will serve as your advocate. If there are meetings required to prove eligibility or provide clarification during the application process, your Case Manager will attend with you. They will help guide you through the process and act as an advocate on your behalf.
We also offer free Medicare Counseling to help you navigate the application process and ensure that you choose the best plan for your specific care needs.
Creating a Care Plan
The Care Team is also responsible for assisting with creating your care plan. A care plan is a living document that outlines all your social and medical care. This care plan is meant to be revisited often and adjusted as needed.
Your needs and care will evolve over time. Your Case Manager ensures your care plan evolves with you and continues to meet your needs. From information and education to help to get medical equipment, we’re here to help organize every detail. Senior case management eases stress around remaining at home.
To create your comprehensive care plan, your Case Manager will collaborate with you, your loved ones, your doctors, and your social workers. Once the plan is made, your Case Manager also documents all meetings, services, and activities, to ensure everything goes smoothly.
Referrals
To ensure you receive the care outlined in your personalized care plan, your Case Manager will refer you to service providers or communicate with them on your behalf. This covers not just medical and personal care services, but any service you may need to remain healthy and protected, including:
- Financial services
- Legal services
- Housing services
- Healthcare services
We can also refer you to Meals On Wheels and/or Senior Companion services, which are designed to further enrich your life. No matter what services you require, we will advocate for you and ensure that you receive the best care possible.
Ongoing Care
As you continue to age and your conditions progress, you will inevitably require higher care. Case management for seniors also helps improve outcomes by ensuring that care evolves with the patient’s needs. Your Case Manager is here to ensure that nothing falls through the cracks and that you receive consistent, high-quality care from all your providers at every stage.

How Much Does Coordinated Care Cost?
All the services provided by our Case Managers are entirely free. While some programs and services to which we refer may require Medicaid reimbursement or private pay, the case management cost itself is nothing.
We will always inform you of any costs associated with any of the services suggested. We are also happy to help you apply for any grants or financial assistance for which you may be eligible.
Does Medicaid Cover Senior Case Management?
As we mentioned above, all case management services are completely free.
