Our case managers are here to help seniors, those with disabilities and others in need. They are trained to match needs in areas such as in-home care, assistance for family caregivers, SNAP, Medicaid, housing, Social Security, Medicare and many forms of assistance that can make families' lives easier. One of our Case Managers will be happy to visit with you about your special needs and the programs and services that will be most beneficial to you and your family.
Meals on Wheels
Our Case Managers help clients sign up for Meals on Wheels provided though our fourteen senior adult centers throughout our ten county service area. Meals on Wheels help seniors remain in their own homes by providing volunteers who deliver healthy nutritious meals to seniors in their own homes. This nationally funded volunteer based program provides the caring support and daily contact that many of these seniors so desperately need.
After our Case Manager does their free assessment they can recommend what programs and services will suit your needs and help you locate the solutions specific to your particular situation. The Case Managers who work at Area Agency on Aging are knowledgeable about the application processes and qualification requirements for various programs and can help with the enrollment process. There is no charge for these services. Case managers work in each office and are familiar with the local community. They are very knowledgeable about the services available in each community that Area Agency on Aging serves.
In addition, Case Managers visit clients regularly to help ensure you have everything that we can provide to help keep you safe and happy in your own home. Case Managers are knowledgeable about the problems of aging, as well as the resources and services that are available to this special generation. Many of our clients are Medicaid eligible, but a growing number pay for their services themselves or have long term care insurance that pays for the care. .
